Release of Medical Records

Northwood Health Systems takes the privacy of our patients very seriously. To ensure patient privacy, a Release of Medical Records form must be fully completed and signed by the patient before Northwood will release medical records. Please note, the completed form must also be signed by a witness and notarized.  Once fully completed, the form must be sent to:

Once a completed release is received, the records are generally available within 30 days. Emergency requests are given priority.
